On-line Forms: US Citizen / Permanent Resident

To apply by mail, please print out and complete the following forms, and mail them along with the US$30 nonrefundable application fee to:

• Information for Residency Classification (PDF)
• Self-Reported GPA Form for Graduate Applications (PDF)
• Recommendation Form (PDF)  (print three copies)
• Graduate Fellowship / Assistantship Application (PDF)

If possible, please include with your application an updated resume so that we may nominate you for a variety of external fellowships.  If you have a mobile phone number, please indicated it in your resume. Inclusion of a resume is optional.

We would also be pleased to send you a set of application forms via post-office mail.   To receive a complete application package, please send us an electronic message at admissions@che.ufl.edu. You may also send us a FAX at (352) 392-9513, or simply write to us at our postal address.  Your message should indicate your complete mailing address and state whether you are a domestic or an international applicant.  In order to expedite the processing of your request, please state your GRE scores (if available), your GPA (in a scale of 1 to 100), and your best estimate of your relative class ranking (for example, 10 out to 30 students).  In addition, please let us know if you have an independent source of funding (such as a government or industrial fellowship).
